Slovakia Passport Documents Checklist
Slovak passports are issued by the Police Force (Policajný zbor) document offices at the district directorates, through the state portal slovensko.sk. Slovak passports are an on-site capture document: you do not bring a photograph — the office takes your picture. Verified against slovensko.sk, October 2026.
The 15-second answer
There is no photograph to prepare. When you apply in person, the document office captures your face (and your electronic signature) on the spot, and fingerprints from age 12. You bring your identity documents and the fee.
Who applies
- Under 15: a legal representative (parent, foster carer, guardian and so on) applies — and cannot be represented by a third party when lodging or collecting the passport.
- Under 18: the applicant applies in person and must attach the legal representative's written consent with their signature. That signature does not need official certification if the representative gives consent by signing the application in person before the issuing authority.
- Every applicant must be present in person when applying, because the face (and, where applicable, biometric data) is captured. The only exception is an applicant who cannot attend through incapacity.
Where to apply
- At the district directorate of the Police Force (okresné riaditeľstvo PZ) for your place of permanent or temporary residence.
- Abroad, at the relevant Slovak consular office; an appointment can be booked online.
What to bring
- Your identity card. If you do not hold one, bring a birth certificate and a certificate or confirmation of Slovak citizenship no older than six months (citizenship can also be shown with a valid Slovak passport).
- Another registry document (for example a marriage certificate or birth certificate) if a registry event has occurred.
- A document issued by the Special Registry if a registry event (marriage, birth) occurred outside Slovakia.
- Your previously issued Slovak passport.
- Payment of the administrative fee.
- No photograph — the office captures your face (and electronic signature) when you apply.
Fees
Paid at application through the eKolok system. Fees below are valid from 1 April 2024 and vary with the processing speed you choose:

A 1-year passport is also offered (12 / 24 / 36 EUR under 16; 45 / 90 / 135 EUR over 16), and a reduced fee applies where a passport replaces a valid one because of a disability record. Applicants who receive a replacement for a valid passport through no fault of their own (for example a manufacturer's error) are exempt.
Tracking your application
You can check the processing status online via the Ministry of Interior, or by SMS or e-mail if you gave a contact at application. When the passport is ready, the document office contacts you.
No printable photo format
