Bangladesh Passport Checklist — Documents & Forms (MRP)

Bangladesh’s passport application paperwork turns on who is applying: certain categories file one form and everyone else files two. You also need an NID or birth registration certificate, and under-15 applicants add one colour photo of each parent (30 × 25 mm). Verified against official sources, October 2026.

How many passport application forms do I need in Bangladesh?
It depends on who is applying. Government, semi-government, autonomous and state-owned organisation employees (serving or retired), their spouses, children under 15, holders whose 5- or 10-year passport is expiring, and people who have surrendered a passport complete one form. Other new applications complete two.

What documents must I attach?
A National ID (NID) card or birth registration certificate, plus attested photocopies of other certificates where relevant (for example doctor, engineer or driver credentials). Attestation follows the official list of authorised attestors.

What is needed for an applicant under 15?
Attach one colour photograph of each parent at 30 × 25 mm, pasted on the form and attested. The child is also covered by the under-15 entry in the table-copies rule.

Who can attest my documents?
The official list includes members of parliament, mayors, deputy mayors and councillors, gazetted officers, public-university professors, Upazila committee chairmen and vice-chairmen, private-university professors, private college principals, private high-school headmasters, national daily newspaper editors, notaries public, and grade-7-and-above officers of semi-government, autonomous and state-owned organisations.

Is a No Objection Certificate (NOC) required?
Where it applies, a Government Order (GO) or NOC must be submitted. Applicants who need one should obtain it from their employer or sponsoring authority before applying.

How do I apply for a diplomatic passport?
Diplomatic passport applications are submitted through the Ministry of Foreign Affairs, not the ordinary passport route.
