The four document cases
Adults
- Cédula de identidad or identity-number assignment (original plus photocopy).
- Up-to-date birth certificate (original plus photocopy) if you are Bolivian through your parents.
Minors
- Birth certificate (original, up to date) and the cédula or identity-number assignment.
- Parents' documents (original plus photocopy); if one parent is in another country, an authorisation by sworn statement — not needed if both parents attend.
Lost or stolen
- Cédula, identity-number assignment or birth certificate.
- A report of loss or theft with the competent authorities (a sworn statement at the consular office may be used instead).
Renewal
- Cédula, identity-number assignment or birth certificate.
- Your expired passport, or one expiring within 6 months, damaged, or out of pages — plus a letter to the consular authority.
Photograph
